Wrasse needs help! No Idea what is wrong

Issue: fairy wrasse seemed health yesterday, was eating fine, etc., this morning he is on his back breathing heavier than normal and doesn't seem to be able to move his body, just eyes and peck fins. He is doing the spiral dance when he can.

Information requested.

1. How old is this aquarium? - 12 years

2. If less than six months old, what is ammonia level? 0.0

3. What is SG of this aquarium? How measured? 1.025-1.026 refractormeter

4. When was the last fish added to this aquarium? 1 year - this wrasse

5. Was it quarantined? If so, how? And how long? Was it prophylactically treated? How? No

6. If you are using a copper based medication, which one? How often do you measure level? When? - NO

7. If you are using hyposalinity, how did you calibrate your refractometer? - No

8. Please describe in detail, the appearance of the fish? If there is one or more pimples, are they lumpy? What color? - colors normal, no marks on the body, lumps, spots etc.

9. Please describe the behavior of the fish as best you can. Is it acting reclusive? Is it always up towards the top of the aquarium? Is it avoiding light? How active is the fish? - was very active and normal up til this morning, hiding at night and active during the day

10. Is the fish eating? What? - yes til this morning - rod's food (several different blends) daily, flakes daily

I have no idea. If he was not on his back he would never know he is in trouble. Any ideas and advise would be appreciated. thanks
 
My only advice is to try to isolate him away from all the other fish if you can. Put him in a QT or in an acclimation box so he's not being picked on.

All other fish look normal? Did you test for ammonia? You sure he's just not in his cocoon? I've had wrasses that act a little disorientated when the lights first come on and they are just coming out of their cocoon.
 
I don't think so. he normally is in his cave in the Rocks at night and swims out when the light come on. thank you for advise. I will check on him again shortly
 
update - moved him to a QT tank. he still looks fine, colors, no marks or spots, he is still breathng heavy and looks like his body is getting stiffer. not sure what to treat him with so starting with PraziPro and see where it goes.
