Abnormalities in acropora tissue

I love bengals
After a long fight (hopefully won, been about 5 months since my last dip) with aefw I have become incredibly paranoid about pests on my corals.

First off is sponges...
I have noticed a sponge like lump on an acro a couple of times. Last time I pulled it and dug around on the affected area and underneath the "sponge" was mushy coral skeleton. The area was maybe a 1/4" or so. Just wanted to see if anyone has had a similar experience or not, or if I should look more into it.

Next is what I have been noticing as raised tissue on the underside of the acro. The tissue will nook slightly bumpy with the same shape as the skeleton. This I noticed on a few smooth skinned acros. All tissue looks healthy in this situation.

Bacterial and fungal issues.
I don't see much talk about bacterial and fungal issues with acros. I am just trying to learn some stuff and would like to know if either of my situations sound like they could be a problem.

I feel like a lot of people don't look at their coral enough, mainly they don't until something is dying or really wrong. So just trying to Avoid that.
