Acclimation process

Hey y'all, I am needing some help. I have a 150g that has been up and running for about a year. I have tried the hanging method, the hosptial tank first, and just putting the fish straight in. But nothing seems to work. Every time I get a new fish it will last me about 2 weeks and die. I know this is vague but my chemicals are correct. The tempature is right. i just can;t figure it out.....
 
What is the hanging method?

Here is how I acclimate:

1. Pour the fish and water into a 5 gallon bucket. Sometimes you may need tilt the bucket so the fish has enough to room to swim.
2. Run a piece of airline tube from the tank to the bucket.
3. Tie a knot in the tubing, get a siphon going and adjust the knot so you get 1-2 drops per second.
4. Let it go 30-45 minutes or until the SG and pH matches the tank water.
5. Bag the fish again and float him for 15 minutes or so to give the temp time to come up to tank temp.
6. Net and release into the tank.

Ive never lost a fish due to acclimation or soon after doing this method.

A good process. However, I never net fish, I use tupperware to transfer them to quarantine.
