angel fish twitching only after prazi treatment?

I have a 30 gal qt tank set up for about 2 months now ,with a 300 display . so I got a good deal on a majestic angel .he was eating like a pig when I got him so I did a prazi treatment for flukes as I do with all my fish. he went through the 5 days fine I di 1 1/2 tea spoons in a 30 gal.
so did a big water change yesterday and he was fine .today I go to look at her and the fish is pale on her head and doing that twitch they do when they have flukes and she was scratching the top of her head on a rock .fish is still eating fine .I am lost here . should I do prazi again?
 
I always do two rounds of Prazi - the second 5-7 days after the first. The reason is because Prazi only kills worms, not any eggs they might leave behind. So you gotta get the "next generation" before they can lay eggs of their own. Thus, the 5-7 day window.

It's possible your angel might still have flukes. Only a f/w dip can confirm that. It's also possible something else is bothering her gills, like Ich.

+1 Once you did the water change it diluted the Prazi and allowed the worms to dig their haptors into the fishes' flesh thus the head shaking and scratching.
You may have to do more than 2 doses.
 
Do another round of prazi then assess his health. It could be wounds from parasites (itchy), or it could be something else.
 
just did a dip and no flukes . I am stumped .

As M.phenax suggested, sometimes fish will display "phantom symptoms" until the tissue completely heals.

Have you treated for Ich yet? If not, I would do one more round of Prazi just to be safe and then treat for Ich next.

How long can this healing process take? I have a DD fish that has been twitching and flashing since the day she showed up, 4 weeks of cupramine and 3 rounds of prazi later still no change. I also did a fw dip to check for flukes and the fish is clean. Sorry op, didn't mean to hijack, but it seems we have similar problems!

Sometimes it can last months. I have a PBT I know had flukes (confirmed by f/w dip). Four rounds of Prazi later, she's still twitching her head. Numerous subsequent f/w dips failed to show any flukes. I go crazy, and start treating her with copper, then CP for what I believe must be Ich in her gills. A friend does a microscopic check of the gills and tells me there are no parasites (even though she's still twitching). It finally ended after 3-4 months, and she's been fine ever since. Go figure.
 
my main tank had flukes . did 2 treatments they were fine. they stopped twitching with in one day and eyes cleared up fine also .
I was stupid to listen to the guy that the fish was clean . I know mine were. so that's why the qt tank now always.

My pink spotted goby had flukes and after Prazi and formalin dips he was still scratching. The lead aquarist at a local marine science center did a fin clip and gill scrape, no parasites. That was 3 months ago and he still occasionally scratches...makes me crazy! I think he does it just to aggravate me;)
 
Interesting... My wrasse has been reclusive since I did the dip, but I'm a bit nervous about moving her to the DT before her flashing behavior stops.

My PBT spent a total of 6 months in QT before all the twitching stopped, and I was confident enough to put her in the DT. Sucks... but I wasn't willing to risk all my other fish.
