Angelfish with Hole in the head disease (Hexamita)

I have a 5" 6-bar angel that had some internal paracites. I treated with metro in a qt. Then I treated with cupramine, and my fish developed hole in the head. After 4 weeks qt the fish went into my display. The fish has been in there for about 2 months now and it appears the hole in the head is getting worse. The fish is eating very well and is gaining weight. None of my other fish have any problems. Any ideas?

There are also some signs of discoloration below the eyes. I'm really stumped as to what is causing this.
I have a qt cycled and ready for him, but he is doing well other than the HLLE.
 
How is your water quality? How often and what size water changes do you do? What size tank?

Do you run carbon 24/7.....There is some research going on about the effects of carbon and it causing lateral line. I run carbon 24/7 and never had a problem. You have to rinse it real good....
 
Water quality is fine. I have some sps frags doing well.
275 display 8' x 30" x 24".
I have about 70 gallons in my sump, and another 100 in my fuge.
So about 450 total (not including rock displacement etc).
I do weekly 55 gallon water changes. All my other fish are fine.
I stopped running carbon about 6 months ago when I noticed some ill effects on my Naso tang. Nothing nearly this severe and my Naso cleared up in a week after I removed the carbon.
 
I had a Blue Hippo tang that I got for free due to the sever case of HLLE however, after feeding it with foods soaked in Selcon, 8 weeks later all was well and good, not to mention that every other fish looked good too.
 
I don't think there is current, I use a grounding probe. How would I go about checking that, a volt meter?

I use selcon, and vita-chem. Feed a wide variety of foods: pe mysis, nori, NSL, shrimp, small chunks of random seafood, angel formula cubes.

This all started after a copper treatment in qt. I read on another thread that cortez angels are susceptible to HLLE, so perhaps the 6bar is also (not many people post about them here on rc). For the time being I'm going to soak all my food in metro. If it continues to get worse I may put him into qt to run a full treatment of metro, or clout.
 
If you are using a grounding probe you should be fine...Unless somehow current is on your neutral. I am not really that good with telling you how to test. Hopefully someone reading this has performed a test with a voltmeter on their tank.....
