Check it for snails on the shell. I have treated clams by pulling them out of the tank and gently scrubbing the whole clam shell under tankwater with a soft toothbrush, then put them back in the tank at the same spot. Do not rub the mantle. I then begin feeding more cryopaste and 80 - 120 micron golden pearls. Also do a partial water change. This sounds a little crazy, but it has worked with me and my friend. I would first also check the calcium and alkalinity of your tank.
