Any idea why chaeto would start falling apart?

Just noticed it's been slowly disintegrating and in some spots browning out...

Any ideas what could be causing this? Also, just has a plate coral die off almost overnight - was very healthy before last water change (or at least I thought)...

It's in refugium, light is on approx 8 hours on alternate cycle as tank.

Give us more details on your system including filtration, livestock and parameters

are you running a new calcium reactor?

somtimes a new and or high jump in calcium can kill it off

From my experience, low nutrients would do it. I ran bio-pellets and the chaeto died on its own because the bio-pellets sucked everything out of the tank.

As far as livestock is concerned I have 2 clowns, small blue tang, mandarin and a potter's angel. Inverts are 4 shrimp and 2 porcelin crabs... I've cut back on feeding due to an algae bloom but with last check my nitrates were a touch high & have since resolved.
