Anyone observed this before?

I'm presently feeding my clams. I have them in a plastic container with phyto. Anyways, one of the clams that has been looking kinda sickly lately, has something sticking out of it. It looks like the bysal foot, but it's sticking out of the intake. It also looks kinda like a very large worm of some sort. Anyone seen something like this before or have any idea what it is?

I do believe it's his foot. I think he wants to move, but IIRC from reading here, that's not a good sign.
