Brook issue?

I am having an issue with fish passing away from brook (sp). First I lost a pair of clowns and now my coral beauty. The only fish I have left in that tank is a sailfin tang that has cloudy eyes. I have tried large water changes, fresh water dips and rid-ich. So far nothing has helped at all. The tank has a few LPS, Anemones and inverts. Does anybody have any ideas. I am lost and do not want to loose my sailfin. HELP!!!
 
Brook is treated with formalin. You'll need to remove your sailfin from your DT and treat in hospital tank with either a long term bath or regular dips of formalin. No fish can go in your DT for a while to allow the parasite to die, I have read recommendations of 4-12 weeks, but I'm not sure how long is actually sufficient.
