I don't normally post here, preferring to collect my knowledge passively by lurking and reading as much as possible. I wasn't able to find the answers that I was looking for though, so I thought I would post.
I just Picked up this new bubble tip last week and has been sitting in a basket since. About two days ago I noticed white mucus coming from the tips of a few tentacles, at first I thought it had grabbed a few mysis during feeding.
Since then its contiued to get worse. I replaced the carbon last night(passively in a bag), and am doing a water change now. I normally change 10% every friday. Tank age 6+ months, w/ 4 other bubble tips doing just fine + 1 Malu doing amazing(Malu was in a basket next to this bubble tip)
Parameters are as follows:
DKH: 9 (steady)
PH: 8.2-8.4
NO3: 0- 2 pppm (Salifert)
PO4: 0 -.003 ppm (Salifert)
Only other post I found was this from 2009:

Here is the bubble tip this evening...Its covering more of the anemone, but it didn't come from the mouth.
