Can someone help me with my clam?

hello,
my clam doesnt look to good, it fell over a week ago and i set it back up but hasnt looked that good since. It kinda awkwardly sitting on a rock, and im not sure if i should move it or not so i was waiting for someone else to give me some suggestions.
heres a pick, oh ya my cleaner shrimp cleans part of it everyday.

doesn't look all that bad to me, I would move him out of the sand and up in the rockwork, assuming this is your 40g with the 1x250W mh and pc suppliments I would move him up higher, probably top half of the tank.
 
I'm not an expert, but it looks like your clam has developed a case of Pinched Mantle. Wait to see what the experts here have to say, but I'd prepare to do a freshwater dip.
 
can you post a top down pic and your water params. when you get home?

does your tang take an interest in the clam?
 
ok I tested everything when i got home and everything was in order except for my salinity. I had a problem with my auto top off and it pumped a bunch of fresh water in once and now the salinity is about 1.023. Its usally about 1.026 and i was just going to raise it by just making stronger batches for my next few water changes. Ive got another pic from the side of the tank, but its too ripley to get one from the top, I think ill just turn the powerheads off for a minute. Do you think that it falling over had anything to do with this?
 
oh ya, my tang does not take interest in it, actually nothing ever bothers it.

check it and make sure there are no pyram snails on it and wait another day. some times the best thing to do is nothing
 
it looks a little bit better today, i checked for thoes snails and didnt see any but might get a 6 line just in case. does it look life threating?
