Had they been through quarantine?

Most wild-caught anemonefishes are probably best to automatically treat for Brooklynella disease to eliminate this possibility.

You're probably right about the internal condition. They can be infected with worms, which is very common, or a variety of other internal ailments. This may give you some of the possibilities:

Since worms are so common amongst wild-caught fish, I would treat for that:

but if your fish are not likely to have worms, then I would treat with an antibiotic as if the fish had an internal infection.

You could do both. Since the deworming agent is fed to the fish and the antibiotic can be administered to the water (like Maracyn Two for Saltwater fish, which is absorbed into the fish), you can do both at the same time. Deworming does not require the fish be moved to a hospital tank. The antibiotic treatment though should be done only in a hospital tank.

Good luck!
