cloudy eyes, stringy fins on flame angel

My flame angel in QT started to come down with cloudy eyes and stringy fins overnight. The next day, yellow tang & copperband in the same system started getting cloudy eyes as well. Today, I found the copperband dead.

These guys have been already treated for over a month with chloroquine diphosphate and prazipro, in separate steps, already.

Any ideas what it could be? At first, I thought flukes but maybe bacterial infection? Scales on the flame do seem raised and "blotchy"

Today, I'm noticing mucous slime over the flame's body and yellow tang found dead...this seems to kill the fish extremely fast. Flame angel also seem to be having trouble swimming upright. Am I also looking at velvet? Brooklynella? What treatment course would you recommend?

Hard to believe velvet or brook would show up after a CP treatment. For brook the best course to take is formalin baths. How is repiration on the fish?
 
That's what I don't understand either. It surfaced after the CP treatment. I was running it at 50mg/g; could it have been too low to effectively treat it?

Breathing is extremely rapid at this time. Are there other symptoms I should be looking for to effectively determine whether it's a bacterial or parasitic?
 
I started the QT back on 50mg/g of CP again before checking replies on this thread. To get it up to 80mg/g, would it be ok to dose 30mg/g worth on top of the current concentration?

I thought I read somewhere that CP loses its effectiveness after the first day and should not be dosed consecutively...but I might have misunderstood.
 
Small update: I'm starting to get red slime algae on the bottom for some odd reasons. Could this be an indication that I'm having a bacterial infection?

Possible to mix CP & anti-bacteria meds?
