Clown fish help

We need more information to help you. How long have you had it? Did you add any fish to the tank? Did you make any changes to the tank in the last 60 days? How big is your tank? Do you have a quarantine/hospital tank on hand? If you need to treat for a parasite, which is my first inclination, you need Seachem paraguard, Seachem Cupramine or some other product of your choosing. Let us know more and we can help.

80 gallon tank.
There's 8 fish total in tank. But all small. She was still the boss of the tank.
Nothing has been added to tank in at least 3 months. Not even coral.
I've had the pair for about 7 years. The male is still fine.
And yes I have a QT running.

Have you ever had issues with ich, marine velvet, flukes or brooklynella in the past? I would be very suspicious of some kind of parasite in the gills which is where they most always go first. Then appear on the body... IME by the time the more progressive parasites (marine velvet and brooklynella) get to the body - white dots similar to ich, chances are slim they will live. Very hard to treat and save them due to the difficulty breathing from the parasites in the gills. Throw on top of that medication. Any other fish in the tank (what kind are they) showing any signs as well? Is the clownfish doing any of the following - swimming into the flow coming out of a powerhead, hiding under a rock trying to stay out of the light? What has happened since your first post?
