Clown Fish not looking good

Can you tell me what might be wrong with this clown fish? At the top of the fish it looks like it rubbed itself raw and now this eye in this picture looks covered. The fish also seems to look paler than normal.

Yikes, poor guy.

Based off what I've been reading and researching, if it looks like it's skin is "peeling off" it may be brook, but I'm sure the experts will be along very soon...
 
I see for the treatment it says to dip in Formalin, I assume this is formaldehyde? Does anyone have experience with this?

Brooklynosis: a parasitic infestation more seen in clowns than other species: if a pale body film, likely; other symptoms lethargy, spitting out food, difficulty breathing: formalin dip: Formalin added at a dosage of 1000 ppm for some 15 minutes. Revive brand med added to water: contains acriflavine---for 14 days. DO not treat in main tank.
 
Yes, it works.
Dosage: 1ml 37% Formaldehyde per 100 liter
It can be combined with malachite green (which can also be used to treat the HT and is even tolerated by many inverts)
After the bath the fish needs to be placed into a tank without the parasite being present to prevent re-infection (which basically comes down to TTM).
Dosing Chloroquine phosphate in the HT may also be a good idea (then I would only sterilize the HT once every week)

I would also treat the fish with antibiotics as secondary bacterial infection are likely at this advanced stage.
