I've never actually seen a picture of one before, but if the WORM is still in the tube, and water conditions for it improve, it will regrow a new one, toss the old one, and start feeding phyto to your tank 3x week if your not already.
A cocoa worm that throws it's crown is not nessisarily dead. You can remove the old crown if you'd like, but don't disturb the tube. You may be surprised in a few weeks when a shiny new crown emerges.
