Coral ID + Care info needed: Browning out

Can someone ID this coral for me and maybe share some tips on its needs?

I had it low in my tank and it started turning brown, so I moved it up.

Newly moved:

One month later, the polyps are big and poofy and I've got a nice growth edge (bad pic - the whole coral actually looks like what you see on the left corner - bright yellow/green and blue):

Unfortunately, part of it was in the shade, and that part turned brown, so I moved it again. For a week or two it started regaining the green color, but now, the polyps are turning brown (or they're completely missing) and the growth edge is turning white:

If anyone knows what exactly this coral is and what I should do, please let me know!

montipora peltiformis..when i had one i kept it about 24" from a 400W halide..too high and it will bleach. flow was medium and varied.

keep mine in bottom 1/3 of the tank, medium-low flow and about 250 PAR :

I did move it up and left a little bit, closer to the center of my light fixture. I'll try moving it again. It seems awfully picky though, like I only have a 4" range to place it in.
