I'm a little disappointed in LiveAquaria.... Surely they were already beat up prior to being in LA's hands?

LA has a fairly extensive quarantine proceedure. Animals go through at least 2 weeks of QT (and sometimes more) before being sold (at least that's what they say). That the mantids would be in such condition at point of sale tells me that they 1. Have not molted and 2. Have not been in QT long enough to do so.
I don't think they would have been beaten up at the facility.

Say no to clove polyps
The second O. scyllarus that I ordered from them was rough looking also. I just didn't know any better at the time and ordered it as fast as I could. Looking back at the picture I could see small spots of shell rot. I contacted them about it and politely said maybe the ones with shell rot shouldn't be "Diver's Den" material. The way they are usually snatched up so quickly I'm sure they put everyone they get on DD.

You think that the shell rot came from the lights they use? In this case don't you think they would have put it in a tank with something a less powerful for a light to prevent the shell rot?

I don't think that it came from the lights. Possibly it got hurt/damage during its capture (maybe)? Then the infections kept on spreading? Actually I don't even know for sure if it is even shell rot.
And I couldn't see them spending a lot of money on lights for holding/quarantine tanks. My assumption would be flourescent lighting for these non coral tanks.
