I have been struggling with sudden deaths for a while now. I have ich in the display which none of the fish have ever shown signs of. It was discovered that I had ich during ttm. The fish in quarantine got it from water change from the main.

So now I'm having mystery deaths. One day not eating next day dead. Last fish added 4 weeks ago.

Fresh water dip nothing. Fish appear healthy one day and dead the next. No flashing or scratching.

Any ideas?

Using old tank water for TTM is something you shouldn't do to begin with.
That said it wouldn't matter much if your system has ich and you put the fish after TTM in there.

As for the deaths - it is hard to diagnose from a distance without good pictures and a detailed description of the fish's behavior.

I kind of doubt that your fish look ok one day, stop eating the next and are dead the following. Likely they are sick and show it.

So please describe how the fish behave
- do they behave different than before?
- do they breathe heavier than normal?
- do they scratch
...

Water parameters are all good on 225 gallon total system (sps).

No scratching or darting observed.
They just become listless then heavy breathing.
Examination of fish seems to show no signs (skin and fins seem normal)
The only thing I can maybe say is fish is a little pale when it is close to gone.
Process from noticing that not acting normal until death usually a day or two.
 
The fish in question (this has happened over the last 4 months).

2 anthias (other 2 are just fine.... For now)

Tank would go weeks without a loss. The. Another would happen.

Last addition about a month ago was 4 lyretail anthias.

Yellow tang
Yellow watchman goby

O clown

That happened to me. I tested and tested my water, couldn't figure it out. Finally realized I had a heater malfunction. Temp was 98F. Sad week for me.
