Deteriorating clown...please help

I've had a gold stripe maroon clown for 8 months now (first fish in the tank) in my 29g reef.

It's gold bands have been slowly fading (last 2 months) to very pale yellow and are now almost white. It's fins are looking more and more ratty, and have been losing pigmentation (has clear spots) at the tips. It has had a few black spots on the fins as well, which seem to come and go. (Each time I'm sure it has black ich and I get the QT ready to treat, they seem to go away...) And I've noticed a few tiny holes on its head, behind the eyes. (HLLE? I thought this didn't affect clowns..)

No pictures at the moment. (Wife has camera.)

My only other fish is a sailfin tang. Clown eats like a pig (even with half of the food it grabs getting placed in its bubble tip anenome). I alternate feedings with mysis, cyclopseeze, formula one flakes, occasional brine every other day.

Water quality is consistent:
80F, 1.023, 0-10 nitrate, 0 phosphate, 8.2 ph, 3 alk, 400 calc. I do PWCs every other week with seawater from LFS. I don't dose with anything but an occasional alk or calc boost to maintain levels between PWCs.

OK, Here's a photo that may help with the diagnosis.
I've circled the "holes" near its head and the ratty looking tips of its fins.
