Disease ID please

This is a picture of the illness. The fish has raise bumps that are inflamed and white The red one is open. The other fish hava a white cottony growth on top of their head. All fish are eating. Its a FOWLR tank. The fish have battled Ick and have treated with No-Ick. Treated with Melifix for bacterial. Premifix for Fungal, now this. Need assistance to ID. Sorry for the poor pic.

Where did you get ammonia poisoning?

Looking at the pic (can you possibily take a clearer one?) that looks like a major growths on his head. Ammonia can kill fish pretty quickly.

How long have you had this fish?
You need to ditch the melifix/p. and get a good antibotic started.

I cant really tell from the pic exactly what I am looking at or what it really looks like, but it looks rather large.
 
You cant get a good picture threw aquarium glass even using the best digital cam. If you can't tell what it is from discription don't bother correcting someone when you can't come up with an answer.You should not need better picture should know buy the discription alone.P.S.. their are aquarium lights on and shinning into camra and from what i see can't get any closer....so if you don't know don't leave comments...

While the picture is not quite good enough for an accurate ID, from what I can see and your description, it sounds like you have a couple of different disease issues occuring at the same time. Ich can still be a part of the problem, IME "reef safe remedies" like No Ick are usless snake oil, and mela/pima fix only slightly better. I would strongly suggest placing the fish in a seperate quarantine tank and treating with a combination of hyposalinity (lowering SG down to 1.009-1.010 over two days) and one of the following antibiotics, Nitrofuran or other furan blend, SW Maracyn 2 or Kanacyn. The hyposalinity will take care of any ich, and the antibiotics are good broad spectrum ones that will take care of the majority of bacterial diseases you are likely to come across in the aquarium.
