Dying Frogspawn?

Obtained this from another member. 4 heads, looked great for 2 or 3 days, but now not so much.

I've target fed it mysis.. not sure what else I can do to save this? Any tips? He doesn't look good, looks like he's really needing something.

Parameters:

pH: 8.2
Alk: 12dKh
Temp: 82F
Nitrates: 0ppm
Nitrites: 0ppm
Ca: 440ppm
Ammonia: < 0.05
Salinity: 1.025 (refractometer)

 
that looks like what my hammer looked like right before it perished.. I bought four heads from blue zoo.. looked great for 2 days and then one guy just shriveled up overnight.. Now the second head is doing the same thing..
 
it maybe adjusting to the light still
what kinda of lights was it under before if it was mh
then going to pcs is a big change
but I've kept that coral under pcs with no problem it was always near the top of the tank though
 
To address above:

This is about 4" from T5 lights (its in my 10g). The powerhead has a deflector to deflect flow to the top of the water to break the surface, therefor there is no direct flow on the frogspawn..
 
Here is my frogspawn. This pic is less then 24 hrs after putting it in. It is in the sand, 12 gallon Aquapod with 150w MH. Everything I have read said that they get most of their nutrition from photosynthsis(spelling). I have tried to spot feed with not much luck, I figure now it catches floating particals around the tank.

I couldnt get the image to load but it doesnt look that great, if they tentacles are retracting like that then it may be adjusting as already stated or near death.
 
my frogspawn looked like that immediately before splitting, and then again about 3 weeks later for just a few days.... Shortly thereafter it picked right up and now looks incredible. just an FYI.
