Hi,

That isn't an egg sack of any sort. I can't tell you what the cause of the lesion is for sure, but it may be related to the chronic Head and Lateral Line Erosion (HLLE) that this fish has. When HLLE affects tangs to the point where there is fin erosion (as in this case) there isn't much that can be done.
I'd guess that this fish also has chronic mycobacteriosis (fish tuberculosis) - and that would be one very common cause for lesions like this. Sorry - but that is not treatable either.

is this something that will spead to the other fish i have? should i get him out of my large tank? I have had him for years and he got the HLLE from a BS rio pump i had that was sending stray voltage into the tank...

thanks

The lesion could be a neoplasm, bacterial or viral infection. It is obvious that the fish has an advanced case of HLLE. You should be making some big changes in what you feed the fish in the way of quality, variety and supplements. I believe that anyting that is a chronic stressor contributes to HLLE. The biggest factors are probably dietary, water quality issues and stress related. Jay may very well be right that the fish has tuberculosis. This would not be surprising since the fish's immune function has probably been surpressed for quite a while now.

i had him and a couple of other tangs in a 60. Since he has been in the 300 he has gotten a lot better...

its not something that will spread? i was wondering if i need to take him out for fear of spreading something...
