Failed 6101

I have a 6101 that will not start up at all with a known good power supply (from one of my other 6101 pumps). I have had intermittent problems with this pump for over 8 months. It used to run intermittently when connected to the 7096 but would eventually fail. Once disconnected from the controller the pump would run at 100% (I believe) and eventually just stopped. This seemed to happen even if the pump was cleaned often in vinegar.

Any thoughts? Thanks in advance.
