Favia Tissue recession

I'm having some major issues with tissue recession on my toxic pools favia. I'm not quite sure what the issue is. I thought it was to low of lighting but I'm not sure. It's a three headed frag and has been in my tank for maybe 8 months. Since I got it the color has faded in the green eyes a lot. I don't have current nutrient levels. But I also suspect maybe they are high though I have started doing 2 25% WCs a week. I run GFO and carbon in a reactor. Lighting is an AI vega color.
