fish ID and problem

Hello,
I have a surgeon fish that I do not know what species is..
I have it for 5 months now.
I have been on a travel and only feed the fish 1 time a day for about 5 days.
When I came back the fish has the fins like if he were on a fight with other fish.
Then the next day I forgot the heater off and the temperature goes down from 25.2 to 24.
The next day the fish has some whites spots..

please if someone can identify the fish and what is going on with him.

White spots could be ich, are his eyes normally very large, i cant tell from the pick, but watch out for "pop eye" as well.
