Flame Angel Died!

Flame Angel started looking weird last night after we got home.
- It was hiding away from the other fish
- Tail looked jagged / fins looked discolored
- Breathing fast & laying on it's side a lot.
- Not Eating (normally a big eater)

Then when the light in the tank went off we noticed that it was floating at the top but when we stuck a net in to catch it, it started swimming again. We took it out of the big tank and put it in a QT by itself. And this morning it was barely moving before it took it's last breath. Don't know if it was ICH or Flukes. We were able to get a picture of it. First time this has ever happened to one of our Flame Angels. We had an anthias a couple of weeks ago that died during the night too without any signs the night before, so not sure if it's all related or something different.

Any suggestions on what this could be? Also since it was in the big tank when it started is there anything that we can treat it with so that the other fish don't get it?

First time this has ever happened to one of our Flame Angels.

How many flame angels do you have, or had? What happened to the others?

Flames can often be a problem and die in the first few weeks/month...how long did you have this one?

We've had 2 Flame Angels... This time we just had one in the tank! Last time our Flame didn't survive the move from one house to another.
 
With the tail looking like that I would expect it was attacked by something... not disease since the rest of the fish appears fine..

So what else do you have in the tank?
 
We currently have 2 anthias, 1 six line wrass, 1 orange clownfish & a cleaner shrimp.

Hmm. None of those would likely attack a flame angel. Any crabs in there? Something like a porcelain or emerald crab could have caught hold to its tail, but it should've been able to escape. What are/were the water params? It could just be bad water quality that resulted in a really frayed tail... The dorsal fins look frayed as well.
