Formalin Dips

Can someone walk me through their procedure for a formalin dip? Length of time, concentration, freshwater or saltwater, etc.
 
  1. fish out of the bag into a suitable size container (ideally clear for observation).
  2. acclimate fish to quarantine tank water.
  3. while doing 2 add small pump or airstone. If needed also provide a heater/cooler or assure otherwise the water doesn't get too cold or too hot
  4. when the acclimatization is done adjust the water level to a known volume (for large fish you may want to account for their body volume and add the appropriate amount of water.)
  5. add formalin: 20 drops (= 1ml) per gallon
  6. leave the fish for 45 to 50 minutes (use timer) in the bath while observing them. Cut the time short if you see that the fish panics or has difficulty breathing.
  7. after the time is up remove the fish from the bath and place it into the quarantine tank.
 
I ask LFS to give me a gallon of water from their tank when they bag a fish. I put that water in a bucket with fish in it. Then I add 20 drops of Formalin. Treatment lasts about 30-45 mins with an airstone. Here's a video of how I do it.

Thanks for the information, super helpful. Do you find all types of fish accept the treatment well? What about very small fish?

I treated small fish this way without issues. The one fish I was afraid to treat and skipped died - likely of velvet. So now every fish gets treated without exception. If it kills them they would have likely died anyway.

Have you ever seen velvet show up on a fish that went through the formalin dip?... not that I'm going to skip my 6-8 weeks of observations after the dip and TTM--just curious.

No.
Though a formalin dip will only be effective enough for a fish with a superficial infection.
If you notice any symptoms that may indicate velvet I would treat with Chloroqine (CP) right away.
