Frogspawn Head Dying?

My Frogspawn has 24+ heads. I've had it for about 9 months. When I first got it, it didn't have much color. It was white, see below:

I was told the lighting it was under was too intense. I about a month it colored up, see below:

It has been doing great except occationally a head with stay mostly closed here and there. I was told they do this sometimes. Now I have two heads that haven't opened for about 1 - 1 1/2 weeks now. See below:

Are they dying? Is there something I should do? My levels haven't changed, my lighting hasn't chaged, so I stumped. Any ideas?
 
How much flow are they in? Looks like they could be getting to little of flow. Just a guess at this point.

Need a bit more info such as lighting, flow, alk, calcium all that jazz.
 
Flow is light, but should be enough. They are at the bottom w/250W XM 10K. Calcium 400 & Alk 8. The rest are doing great. Should I be worried about the two?
 
Hmm pretty similar to the placement of my frogspawn, except i have more flow.

Do you think anything would be picking on it maybe at night?
 
I don't think so. The only thing I know of that comes out at night is my Coral Banded Shrimp, besides the snails & BL hermits.
 
Update. The 1 head completely went in. In the next couple of days a clearish puss started appearing on the head. I blew it out with a turkey baster and it had completely disinigrated.

Only 1 other head in not opening, the rest are fine. I'm not sure what happened.
