Frogspawn melting away

As the title says - All other corals are doing quite well, including a torch and a hammer that sit nearby, but I've lost two of three heads on my frogspawn - they just sort of melt away. No other corals have been affected in any way that I can see.

Tank:
standard 4x2x2 120 with 40b sump
eheim 1262 return pump t'd off to run carbon reactor also
~2" fine sand, ~ #75 LR
ASM G2 skimmer
2 x AI Sol blues running at 40w/65b/65rb 12hrs/day
4 x koralia 3 (1150gph) on smartwave timer to randomize flow

Corals:
~ 20 assorted SPS (Acropora/Montipora/Chalice)
Duncan w/12-15 heads
Large elegance
good sized Acanthophylia
small rock with 6-8 palythoas
orange hairy mushrooms
blue mushrooms

Fish & Inverts:
Orange back Fairy wrasse
Flame angel
mated pair YWG
orange spot sandsifting goby
3 x Bangaii Cardinals
2 x skunk cleaner shrimp
3 x emerald crabs
Harlequin serpent star
Abalone
Common urchin

Water Parameters: (checked 15 minutes ago, Salifert kits
Salinity - 1.026 (by refractometer)
NH4 - 0
NO3 - 2.5 or less
Ph - 8.2
Ca - 450
Alk - 9.3
Mg - 1350

Any and all help appreciated!
 
It's about 6 months old in my system. All other corals are thriving, water tests as close to perfect as can be expected (see parameters above). I doubt it's water quality. If it were, the SPS would be having issues as well. I spoke to Aaron and Chris at Phishybusiness about it yesterday. I moved it away from the torch, and will be keeping an eye on my Flame Angel.
