Green Wrasse possible worm problem

Not been on here in years ... just need a bit of advise... So i have a good sized green wrasse in my marine tank. Upto a few months ago he was a supper healthy happy fish.. ate bristle worms and kept on top of the mini shrimps that had become a problem in my tank. Over the past few months he has been slowly loosing weight. Hes now way too skinny for my liking and has a chubby stomach area. all other fish in tank are healthy happy and a good weight. Ive taken to giving him and extra feed of mysis in the afternoon but its clearly not enough. Could he have worms? hes been in the tank for 4 years and is full size. If so what can i use to treat him. Im in the UK and have mixture of soft corals clan up crew.. mostly hermits and snails so i need something that is reef safe. Or any ideas what it could be.
 
Is it eating well and still not gaining weight? What else are you feeding besides mysis and how often. A varied diet is key in keeping healthy livestock.
Have you noticed any white stringy poop?
If it's eating well and losing weight it might be internal parasites in which case best corse of treatment is metroplex added to its food along with focus for a min of 10 days. You use 1:1 ratio of metroplex and focus to one ts of food.
