Hammer Dying

Hey people,
I have a two headed hammer frag that I've had for a couple months now, after first introducing it to the tank it opened up great, looked good, lately it's shriveled up, won't open at all, receding from the skeleton a bit. I've tried different flow rates near it, different spots in the tank for lighting. Nothing seems to work.

I've had problems in the past with the calcium heading towards the 600 but with no effect apparent on alk. water changes once a week seems to keep the calcium in check. I'm at a lost here, any advice.

My frogspawn, mushroom, zoo's, and button polyps all look great, but the hammer and candy canes are shrivled....well, one polyp on my candy canes looks good.
 
No 'BIG' swings, the s.g. usually runs around 1.023, I top off the system everyday by hand when I get home from work, about a large Subway cup and a half everyday. probably runs around 1.024 in the evenings.
 
Mine was really picky about where I put it in my tank. Even though they're not supposed to be really high light corals, I killed my first one and almost killed the second until I decided to put it up in the top 1/3 of the tank. It's been extremely happy there. The one I have also doesn't seem to like too much flow. Just enough to gently wave its tentacles around seems perfect, and nothing laminar, so you may want to find someplace where the flow is low to moderate and not uniform.
 
Island Crow, I'll give that a try, I havn't tried putting it up too close to the top yet, everything else in the tank, even the open brain seems to thrive in the lower half. Plus the entire upper half of my tank seems to get higher flow, but I have a few ideas for that. Thanks guys
