Hammer Explosion? ..& not in a good way

This Hammer has been acting up for the past two weeks, never fully expanded with short, lifeless tentacles...however it NEVER lost it's color, and was still eating! Everything I researched told me it was splitting so I left well enough alone..until today I came home to this. POOF gone. Now I've had my fair share of issues starting out in this hobby, and creeped through countless hours of forums / pictures...never have I seen anything like this!

Flow/Lights/Params/Stock - Nothing has changed whatsoever, had a short-lived and minor phosphate problem..Phosguard remedied quickly.

Anyone have ideas about what on earth has happened here?

I woke up last week to notice a receding edge of my centerpiece torch, figure I would have to remove that head when I got home from work. Got home, too late, looked like yours except it had about 12 heads! I feel your pain!

Now I'm paranoid that if it can happen once without me knowing why..it can surely happen again!
 
I see cyano on the rocks..
Its also possible that it simply injured itself on its own sharp skeleton simply from too much flow..

FYI.. Its also not a "soft coral" but rather a LPS coral..
 
That particular hammer has been in a very low flow area since purchased so that couldn't possibly be the issue....& the cyano was from the phosphate spike a couple weeks back that the janitors haven't gotten to quite yet.

I'm still at a loss...lol

post a full tank shot, I bet your phosphate spike is what caused it to die
 
Attached picture is from 4 days ago (currently "supposed" to be working so I can't get one right at this moment)

...this is exactly 3 days before the hammer died. It's the small purple-ish one directly above the frogspawn - when I thought it looked as if it was splitting.

Mine did that when my nutrients were too low. First went the hammers followed by my torches and than my duncans. Stopped running any GFO and started feeding a bit more and all is well and showing good growth.
