Hammer Has spots that are moving?

Can anyone one tell me what the heck these are? they move around. I hope there not mushrooms, and if so what the heck do I do to get rid of them all? HELP!

If they are brown and flat looking, then most likely they are flat worms.

I think it's flatworms. They're sort of a triangle shape.
