Help! ID Clown fish disease? What to treat with? (pic)

one of my clowns developed I think it's a fungus or bacteria infection.( it really doesn't have the appearance like cotton but more like the fish was dipped in flour) I am total unfamiliar with treating SW fish. Any suggestion on what I'm looking at and how to treat it?
I have a QT tank set up but all I have in the house at the moment is Kanacyn (A full spectrum antibiotic) and 37% formalyn. Can I use the formalyn in the qt tank @ 1 drop/gal or do I have to use it as a dip? Should I use it at all?
What med should I use?

Looks like the brooklynella, aka clownfish disease. Formalin baths work best. Use 1 ml of formalin per gallon of SW, aerate well as formalin drives out O2. Leave fish in bath for up to an hour, no more and less if the fish starts having difficulty with it. Dip the fish every other day, for a total of 3 dips. Of course it is best to keep the fish in a QT between treatments.
