Help ID Desease

I have a QT with a few fishes including this Emperor Angel. Unfortunately, I got some desease and I am not sure what it is. The pics are not the sharpest but I hope you can help. The fish is looking Ok one day and the next they are dead. Mostly heavy breathing and the white spots. That's why I thought it might be ick, but then the fish has slime coming off its skin and I don't think this is a sign for ick.

I Have Paraguard at home - does Paraguard help for this?

Formalin dips are the course of action for brook. You can use Quick Cure as well. I've not used Paraguard but it does not contain formalin so I don't know if that'll work. Time is of the essence as you noted this disease kills fast.
