Help in identifying worm killing my frogspawn

Hi All,
Hoping to get some help with a problem which I am experiencing a second time. I've found what appears to be a clear worm covered in sand that burrows into the heads of the frogspawn completely killing the branch it infects. The 1st one was kind of small and killed 1 branch. The latest, 2 branches and like the other one, extended from the live rock, up the stalk, then directly into the center of the head. On both occasions I've pulled it off/apart with needle nose pliers. The incidents were approx. a year apart. I've included pics showing some of the worm (?) and devastation. Any help would be appreciated. The offending party is the white tube extending between branches.

It's hard to tell from the picture but it may be a vermethid snail. I would cut off the dead parts so it doesn't pollute your tank, and then I would do a coral dip to kill any pests.
