Did you quarantine it first? They are known ich carriers, always quarantine your fish before you place them in your display tank. Do you see any signs of ich or other parasites?

Nothing it was in QT and I just added it to my DT tank it was in QT for a month

It could be a parasite infection as said about. But Tangs are very strange and Comical at times . Wait until it starts scratching your tank with your fav Coral Frag...

it's better to treat with praziquantel and tank transfer method, then observe for one month in qt when dealing with tangs imo.

One month really is not long enough to QT, especially a tang.
