Generally, I'd say that toads just go through slough off periods. It was common for mine to close up for a month or more, while sloughing off. It also developed white spots and holes that eventually turned into self propagated frags. All in all, no big deal. Just looked rough for a while.
I've had some do something similar to what yours has, but not with as many spots. You are probably okay, but if it starts turning black and slimy in spots, it's going. I've had some melt down over the years, too (though, they usually left something behind and restarted).
How long have you had it? How old are your lights? What is your water quality? How long's the tank been set up? How much flow is hitting it? What fish do you have in the tank with it? Do you RO for top off/wc water? If not, what is your TDS? Just a bunch of general questions that can help point or negate the culprit. Of course, there are often no definites in this hobby. Anyone speaking in absolutes is bound to eat foot fairly often.
