Help with Clam

I was wondering if I could get some help. I have two clams in my tank and one of them seems to have about 4 spots on it that look like snails growing right out through the shell. I can't pick them off as they seem like they are a part of it. They part inside the opening seems to move in and out when I go near it. Any suggestions on what they are and how to get them off without hurting the clam. Thanks Ian
