Help with New fish Dying

I have a 175g reef system that has been up and running for about 1 1/2 years now. Fully stocked with sps, lps and sof coral. I keep adding new fish and they seedm to die after about a month or so.

I recently had a kole tang and a potters angel for about 1 month in my frag tank. They were doing fine ie eating looking healthy. I then added them into the main tank and about 3 weeks later they both died.

I have lost other fish as well over the last year- sailfin tang, another potters, anthias, and about 2 blonde naso tangs before the current one.

I am starting to think that one of the current fish is killing them.

Yellow Tang- in the tank from the beginning
Blonde naso tang
Blue hippo
Bicolor Pseudochromis
Pair of Clown fish
6 line wrasse
yellow tail damsel

The blonde naso was the last fish added after the damsel.

Anyone have any thoughts or have been in this situation?????

Everything is great. The current fish look very healthy and eat like pigs. This is why I can not figure out why I keep losing new fish. I have noticed the Yellow Tang swims near the new fish and raises his fins but it does not seem like it stops the new fish from eating.
