Help with sick/Injured Clownfish

I have a marron stripped clown that has developed two for lack of a better word holes about 1/4 inch to the rear of each eye. They are on each side in about the same location. I am puzzled as the tank is a mature 44 gallon pentagon that only houses the clown fish, a snowflake eel and a few inverts.

Im not sure what to think. There have been no new additions and all water parameters are fine.

The holes look like flesh is missing and they are whiteish in color about half the diameter of a pencil eraser. The fish is not acting abnormal at all. He's eating well, breathing normally and not scraping on rocks.

Any advice or suggestions will be greatly appreciated
