Hippo problem

My hippo looks as if his entire head has been descaled, happened overnight. I am also noticing my cleaner shrimp trying to get to it constantly. There have been no changes in his diet for the past yr and a half. I used Flat worm exit for the first time ever Monday. Can anyone assist, what's going on, what do I need to do, etc...

He is eating and none of my other fish are exhibiting any type of reaction to the flat worm treatment.
