Hippo Tang Ich & HLLE?

Just wondering whether my little hippo tang has ich or HLLE or both? The white spots towards the back look like ich. The black spots are actually translucent white in real life. Additionally, the yellow area near the tail have what looks like burst blood vessels, sort of a blotchy reddish color. I have her in a QT for the past 4 weeks and the ich spots are still there. The SG is at consistently at 1.009 and this does not seem to be working. I had added some carbon in the hanging filter but have removed it now.

Anybody have any new ideas about what I can do? Thanks!

Well just to close off this thread, I'm happy to report that my little hippo tang and all his friends are ich free after 6 weeks hypo and then 2 weeks cupramine. They're happily swimming, eating, and playing in the tank for the past several weeks, with no signs of ich. Woohoo!
