Hippo trouble

I received a yellow belly hippo Sunday that had been at lfs since Christmas. It's about 3-4 inches.

Yesterday I couldn't help but notice this off blotch on his one side. Looks as almost if it's the size of a dime or nickel, grayish in color, and raised, sorta like when someone's gets one of those burn tattoos, where it raises from the skin. (Some people get the horseshoe burned)

I noticed he grinds that in side, in particular that one spot, against a rock. No signs of ich, once in a while a head shake. Not sure if it's irritation and should be left alone, or some sort of infection.

Also Hippo tangs are known for wedging themselves under a rock when they sleep or scared which can cause a slight white discoloring on their body if they scraped it.
Head shaking might be sign of flukes.
