Hmmmm...what do you guys think?

Well I have a recent picture of my Condy. I just took it like 20 minutes ago. Here it is:

From the single recent picture and descriptions, I would chase water variables and increase the feeding frequency but reduce portion size.
 
IMHO it is most likely unhappy with something in the water. Either an additive, or a build up, or missing something, or a day/night fluctuation, etc., etc.

There is also the chance it is unhappy with the diet, but I would expect the tentacles to be shorter if that was the case.

Seeing the pictures, I agree with Scott. (edit) I had not realized that it managed to shrink that much(/edit) When I noticed my anemone going into an unknown decline about 5 years ago, I changed a significant portion of the water (~50%) and the problem stopped. The anemone promptly recovered, etc. So whatever it was, it was in the water (or not in the water, but I find the former to be more likely). If all else fails, do a substantial water change.
 
Hmmm I use oceans blend as my additives. What would be missing in the water?

I feed pieces of shrimp and fish and sometimes it gets flakes so the diet is fine.
 
What kind of additives? Most coral folks I know only use a 2 part calcium/alk additive. Well, that and food stuff.
 
Thats exactly what I am using a two part additive hence oceans blend as I stated above that I am using. Oceans blend is a two part additive. Very good one at that..
