How to use amqel plus in hospital tank

I'm doing tank transfer method for eradicating ich. Do I put amqel in with the freshly made saltwater or do I put this in when my ammonia goes up?

I don't see why anyone would put it in with freshly made water unless it works constantly instead of instantaneously.

I just put in 6ml since my ammonia went to .25 but does it continue to work or of I dose daily?
