Give your parameters. Nobody can help without knowing your tank size, age, water source, water quality [in numbers] and what's in the tank in some detail. Most likely water quality culprit: nitrates/ammonia s/b 0; or alkalinity s/b 8.3-9.3.

Have you inspected for some sort of predation?

I had a bacterial infection but it just wiped out the heads.......its recovering nicely now.
