If a supplier keeps copper in his tanks, and I buy a fish from them that has been quarantined for 45 days, is there anyway that fish can have ich? If so, how?
 
one word... TRUST! I take someone who stands to benefit financially from an arrangement's word with a grain of salt.

do you TRUST the supplier?
do you break bread with that supplier?
would you give him the keys to your house? Your car? Your credit card/bank account numbers?
would you allow him to babysit your children?

EVEN if you answered YES to ALL those questions, I would still Quarantine fish from him.

I LOVE my family. My uncle got me into SW fish as well as acted as a father figure to me for much of my life. I worked for him as a teen and due to what he has taught me in that time I have a career I never thought I would. I owe him a debt I could NEVER repay. I would answer YES to ALL of those questions I posed above. But I also know that he has had ich in his tanks even though he doesn't APPEAR to now, and for that reason alone I would even QT fish I got from him.

If you normally quarantine your fish, I would keep doing it. If your supplier adds any fish to those tanks (I am sure he does) in those 45 days, he would be adding more ich.
 
Most LFS don't run Cu at a level high enough for a 100% ich kill. Diver's Den, perhaps the best source of fish on the planet, runs Cu 24/7. they strongly recommend that you use a QT with their fish. If a lfs runs copper and has flame angels in the system (as DD often does); you can bet that Cu isn't at a level high enough to kill ich. The FAs are very Cu sensitive. Cu also kills algae and countless other organisms; a big reason for dealers to use it.
Fish often go through some LFSs so fast that buried ich, which is not killed by CU, will still be there when you buy the fish. If ich shows up in your QT, its fairly easy to treat. Velvet or brooklynella, both can slip through the light copper defense, are far more dangerous.
