I tried posting in another forum, but did not get convincing answers. Please tell me what disease this Lionfish has:

Looks like ich to me. Why don't you think it's ick?

Because I had him in QT when I purchased him and he showed no signs in that period (about 1 month). Furthermore, he is in a tank with only a zebra eel, leaffish and frogfish - all who went through QT without any signs of Ich.

Many of these white spots seems to be not really attached to the fins but are only loosely attached to it - swaying independently of the fins... It just does not make sense.

It was a new setup - new RO/DI mixed water, brand new tank...

Ich also looks like small bumps, they dont look like patterns. I think your lion has ich. You cant be 100% sure about it by keeping the lion for 4 weeks in your QT. I would recommend you to take the lion back to the quarantine if its in the main tank.
